In a study of 770 full-time working mothers in Southern California, drawn from a case-control study on preterm birth and low birth weight, researchers explored how maternity leave impacts breastfeeding success. Conducted between July 2002 and December 2003, the analysis revealed that postpartum leaves of six weeks or less quadrupled the odds of failing to establish breastfeeding within the first month, while leaves of six to twelve weeks doubled that risk, compared to mothers not returning to work. Short leaves also hastened breastfeeding cessation, especially among nonmanagers, those with rigid schedules, and women under high psychosocial stress, as shown through multivariate regression models adjusting for sampling probability. Antenatal leave showed no effect. With California offering rare paid pregnancy and bonding leave, the findings underscore postpartum leave’s critical role in supporting breastfeeding, urging pediatricians to advocate for extended, flexible policies to bolster maternal and infant health.
